School Based Health Center Case Management Specialist Position available at International Community Health Services’ Seattle World School location.
The SBHC Case Management Specialist provides case management to youth (ages 11-21) who access services at ICHS’ School Based Health Centers. Case management services include, but are not limited to: assessment of health goals and barriers, care planning, referral linkage, health promotion/health education, and individual and family support. This position engages in coordination of primary medical, dental and behavioral health as well as specialty care and social needs services and resources. Competencies should include strong organizational skills, excellent interpersonal skills, knowledge of complex systems, and ability to manage a caseload of up to 20 patients.
